About Us
Planet PCP was created to assist Regional Center clients with the development of their Person-Centered Plan (PCP) as part of their enrollment into the Self-Determination Program (SDP).
Planet PCP's mission is to equip persons with intellectual and developmental disabilities (I/DD) with a Person-Centered Plan that will act as the foundation to build their capacities and capabilities. The PCP will help guide individuals towards their preferred futures by informing the Initial Budget, Spending Plan, and Individual Program Plan (IPP) process under SDP.
Services Offered
Initial Person-Centered Planning
Complete our initial referral form. Once submitted, we will review and you will be contacted via email.
Contact your Regional Center Case Manager / Service Coordinator to let them know you are working with Planet PCP to create your Self-Determination PCP. Your Regional Center will provide all funding necessary to create your PCP. We request a standard collateral packet to move forward with your PCP referral.
Expect contact from Planet PCP once collateral is reviewed to set up a meeting. In-person meetings available in the Bay Area, Zoom video conference is preferred if you live in Southern California.
Hold a meeting with us to develop your PCP, discuss why you're enrolling into SDP, and identify unmet needs from traditional services.
Receive your PCP within 30 days of the meeting and move on to the next step of your SDP enrollment!
Please note: Planet PCP does not offer Independent Facilitator services at this time. We are not able to assist with the development of your SDP Spending Plan.

About the Founder
Having over 5 years of experience serving Regional Center clients, my passion for working with the I/DD community began in 2018 while working in an adult tailored day program. I have since worked as a former Service Coordinator with direct experience enrolling individuals into Self-Determination. Currently, I am studying to attain a Master of Social Welfare (MSW) degree at the University of California, Berkeley.
